What Can I expect from Cognitive Concept's Program?


 

We provide programming that includes basic structured daily activities that meet the person's needs (vocational, recreational, therapy, counseling, etc.), crisis management plans and emergency plans, and work as a team with those involved in the person's programming (county case manager, family, doctors, other programs, etc.)

If a person is placed outside of their home community- and one of the identified goals is to return to their community, CCI will strive to make this happen. Programming will be focused on stabilization of behaviors and addressing those skills that may be challenging community placement at this time. When appropriate- and when the team is in agreement- return to the home community will be initiated. CCI will work with County representatives to identify an existing resource to transition to, or CCI may be able to provide ongoing support in the new location. In either scenario, all efforts will be made to make sure that support staff or family are trained in techniques that have made stabilization possible.
