Referring individuals

Simply call our office and ask for Jodi Leverenz or Gail Schmidt. We will ask some basic questions over the phone and would appreciate your forwarding pertinent information to our office. We will make arrangements to meet the person where they currently reside and complete a basic assessment. We will then get back to you with program outlines and estimated costs and staffing patterns.

We are interested in development of programming throughout the Northeastern part of Wisconsin, and would work with you in determining the details of location and appropriate placement of the person. Sites include supervised apartments, adult family homes, and other creative situations.
